FY2004 ODJFS 9400 Submission Deadline

On August 8, 2005 the Office of Ohio Health Plans (OHP) issued a letter outlining the timeframe for the Combined Proposed Adjudication Order (CPAO) for the calendar year 2002 cost report period (state fiscal year July 1, 2003 – June 30, 2004). The OHP plans to issue CPAO's in February 2006.

OHP is requesting that facilities submit Nursing Facility Payment and Adjustment Authorization forms (ODJFS 9400) for the July 1, 2003 to June 30, 2004 period by September 16, 2005. Providers should not re-submit any ODJFS 9400s previously sent to the department. Any 9400s received after this date will be returned to the provider unprocessed. Providers will need to include any unprocessed ODJFS 9400s during the exit conference.
